RN Discharge Coordinator

HealthCare Support is actively seeking an RN Discharge Coordinator to fill an opening with an acute care facility in Brownwood, TX.

Daily Responsibilities for the RN Discharge Coordinator:

• Act as the primary point of contact on units, fostering collaboration among Nursing, Case Management, Physicians, and Patients to ensure the following tasks are completed prior to discharge.

• Ensuring a seamless transition for patients at they move through the continuum of care.

• 5 x 8's (Monday - Friday)

Required Qualifications for the RN Discharge Coordinator:

• Texas RN license

• Minimum of 3 years of bedside RN experience

• Minimum of 2 years of Case Management Experience

• 6 months of Charge/Leadership experience

Pay Details:

• $29.00/hour - $40.00 an hour

• Nights, Weekends, Holidays Differential: $3.50 an hour
