Job Title: Student Support Transition Coordinator
Purpose Statement
The job of Student Support Transition Coordinator is done for the purpose/s of being the liaison for students re-entering Campbell County School District, from the YES House, Behavioral Health Services, or other hospitals, treatment centers, or alternative school placements. This specialist will work collaboratively with principals, school counselors, school psychologists, and families/guardians to devise action plans that advocate for the needs of the transitioning student, while also considering the safety of all students and staff. In addition, this position will help coordinate and implement programs and resources to positively impact transitions between elementary and junior high, and junior and high school students.
This Job reports to Assigned Supervisor.
