Responsibilities and Duties:
Assist individuals with developmental disabilities in finding services and supports in their community
Draft and revise Individualized Service Plans that focus on the individual’s goals and aspirations
Find service providers in the community to meet an individual’s needs
Check to make sure the services are meeting an individual’s needs by performing monthly meetings
Keep record of all notes, documentation, and reports
Answer questions on service providers and community involvement
Respond to emergencies
Qualifications and Skills:
Support Coordination experience is REQUIRED
Bachelor’s Degree (no preferred major) REQUIRED
Minimum of one years experience working with individuals with developmental disabilities REQUIRED
Great communication skills (oral and written)
Organized and able to make and hold your own schedule
Computer proficient and well learned with Microsoft Suite
Creative problem solver with great research skills
Knowledgeable about the county you live in and the surrounding counties
Must be a valid licensed driver and have a personal vehicle that is registered and insured
