Director of Social Services – (Long-Term Care)
We have a great opportunity for an experienced full-time Director of Social Services to join our management team. Ideal candidate will be an MSW/LCSW with long-term care experience. Familiarity with CMS regulations, MDS assessment, and annual DPH survey process. Ability to appropriately handle the emotional needs of both our residents and their family members is essential.
Responsibilities include, but are not limited to;
· Ensuring resident adjustment to rehab or long-term care by performing psychological screening, initial care planning, introduction to services and procedures and completing all necessary paperwork.
· Participating in admission process, grievances, MDS assessments, counseling, IDCP meeting, d/c planning.
· Following policy and procedures, federal and state compliance as well as leading by example.
· Long Term Care Experience 1-3 years
· Current Massachusetts License in Social Work
