    Title: Administrative Navigation Support

    Hours: Full-time, 37.5 hours/week, daytime hours

    Brief Description:
    Provide support and outreach in the community to form relationships with individuals, businesses, and
    nonprofits in eight counties. Provide service linkages such as assisting with housing, food security,
    Medicaid application, and services to impact the social determinants of health. Participate in data
    collection and entry for measuring program outcomes. As a member of the CSSMV Front Door/Ohio
    Benefits department, participate in monthly team meetings, quality assurance activities, and agency
    activities.

    Key Responsibilities:

    • Assisting consumers, informal support, and County Departments of Job and Family Services
    (CDJFS) by contacting regarding needed information and status of Medicaid application
    • Completing home visits within DJFS timelines to assist consumers in compiling information needed
    for Medicaid application
    • Data Entry of Mon Ami system and assisting administrative staff with the reports as needed
    • Entering information in Ohio Benefits and Provider Gateway portals
    • OSHIIP certificate training, data entry including tracking, and community and staff education
    • Data entry and Ohio Benefits navigation
    • Serving as a source of information and referral for programs and services, and referring to
    appropriate agency
    • Checking OBLTC for Medicaid status and instructing individuals on Medicaid
    • Providing Case Aide work for Care Coordination programs, Shelby Senior, and new programing
    • Performing clerical duties as requested by Director or Assistant Director

    Minimum Requirements:

    • High School Diploma required; Associate degree preferred
    • At least two years of experience or training or equivalent combination of education and
    experience
    • Ability to use a computer with substantial ease, including Word, Excel, and Outlook
    • Ability to complete home visits, which requires reliable transportation
    • Good communication skills
    • Ability to work with diverse populations
    • Willingness to work some flexible hours
    • Knowledge of Medicare and Medicaid preferred
