WHAT YOU DO:
As the Admissions Coordinator at the Luther Acres Healthcare Center, you direct, conduct, and coordinate the admissions process for residents, prospective residents, and families to ensure efficiency. Among your responsibilities, you will:
Identify and screen prospective residents in both the acute and community settings and respond to inquiries by potential residents, family members, or placement agencies.
Work independently to schedule and handle all tours, meet with potential residents and their families to determine their needs, provide solutions to any issues or concerns, and plan for move-in.
Work with residents and families on the beginning stage of admission process to address resident needs. Act as a liaison between the Healthcare Center staff and family members during the pre-admission and admission process, and following admission, refers family members to appreciate managers depending on specific need. Organize welcome meetings for all new resident admissions.
Be responsible for ensuring the completion of the admission paperwork with the responsible parties upon admission and ensuring the admission packet is completed in a timely manner and that all forms are present, neat, and organized.
Maintain effective relationships with hospital discharge personnel, administrators, other healthcare centers, physicians, clinics, rehabilitation centers, and home care agencies.
Conduct follow-up calls upon discharge as directed, needed, or determined by resident experience.

Luther Acres – located in the charming, historic town of Lititz – is a Continuing Care Retirement Community with a long history of providing excellent care and services. It is part of Luthercare, which is a not-for-profit, faith-based, and mission-driven organization. Luthercare provides a network of services through its full continuum of care with three communities in Lancaster and Lebanon Counties. Luther Acres is its largest community, with nearly 600 residents living across its independent living cottages and apartments, personal care apartments, and skilled nursing healthcare center.
QUALIFICATIONS/REQUIREMENTS:
A valid PA driver’s license
Two (2) years’ experience in a customer service-oriented profession
Two (2) years’ experience working directly in admissions, marketing, and/or sales.
Working knowledge of Medicare/Medicaid, managed care referral process, reimbursement programs, long-term care regulations, and admissions and discharge procedures.
PREFERRED JOB-RELATED EXPERIENCE/ABILITIES/EDUCATION:
Basic understanding of medical conditions, medications, and functional status for the population related to skilled and rehabilitation services.
Bachelor’s degree in a related field of study or equivalent combination of education and directly related experience is preferred.
