Team Alpine is looking for a Full-Time Social Services and Admissions Coordinator
Position Purpose
Participates in all administrative and operational activities of the Social Services and Admissions Department in accordance with current federal, state, and local standards, guidelines and regulations, and the facility’s established policies and procedures.
Major Duties and Responsibilities
The Social Services and Admissions Coordinator will assist the Director in census development and occupancy management to achieve financial goals for the facility. The coordinator will work with the Director on identifying opportunities with referral partners and develop strategies to build relationships within the local community. The coordinator will work with the Interdisciplinary Team at the facility to ensure needed documentation is obtained for all admissions before and after entering the facility. Works with the Director and facility team to determine placement of admissions, review of insurance policies and medical needs for all admissions.
The Social Services and Admissions Coordinator is responsible for assisting residents in attaining or maintaining their highest practicable well-being. Coordinator will assist to complete the social services portions of the comprehensive assessments. Coordinator will assist Director with QUAPI projects as requested. The coordinator helps to ensure that staff members are knowledgeable about Resident’s Rights and encourages staff to maintain and enhance each resident’s dignity in recognition of each resident’s individuality. The Coordinator will also advocate for residents and assist them in assertion of their rights within the facility.
