Adult Day Care Director creates and directs programs that address the health, psychosocial, and educational needs of older individuals and their caregivers and that provide clients a safe, secure, and positive daily experience. Responsible for recruiting, training, and retention of qualified staff. Being an Adult Day Care Director manages the financial operations for the center including billing, reimbursements, and budgets. Performs assessments of prospective clients to determine if programs will meet their needs. Additionally, Adult Day Care Director coordinates with caregivers, families, social services agencies, and medical professionals to plan clients program of care. Monitors programs to ensure effectiveness, quality care, compliance with applicable regulations and standards, and to identify areas for improvement or expanded program offerings. May be a RN with experience in gerontology. Requires a bachelor's degree of social science or a directly related field. Typically reports to top management. The Adult Day Care Director typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments. To be an Adult Day Care Director typically requires 3+ years of managerial experience.     Kitsap Mental Health Services (KMHS) is seeking a dedicated Care Coordinator with a Bachelor's Degree in Psychology, social services, or a behavioral health field to join our Adult Outpatient Team. As a Care Coordinator, you will be an essential member of our multidisciplinary team, providing comprehensive care coordination, treatment, and support services to our clients.

    Primary Responsibilities: 

    • Provide direct care coordination services to an assigned group of clients, including individual, group, and family counseling, crisis intervention, and case management.
    • Collaborate with the treatment team to develop, implement, evaluate, and revise treatment goals and plans for clients.
    • Educate and support clients' families, advocating for clients' rights and preferences. Coordinate with community agencies and systems to ensure holistic treatment.
    • Assist clients in finding primary care physicians, dentists, and coordinating psychiatric care with medical care.
    • Offer crisis intervention counseling and collaborate with Designated Crisis Responders when necessary.
    • Maintain thorough and accurate documentation of client progress according to established policies.
    • Participate actively in organizational meetings and treatment planning review meetings.
    • Provide information and support to clients regarding substance use issues, mental and physical health, and daily functioning.
    • Help clients access resources and education for increased autonomy and meeting basic needs.
    • Monitor and facilitate Legal Restraint Alternatives (LRA) compliance for assigned clients, ensuring continuity of care.
    • Collaborate with the Crisis Response Team (CRT) and court-involved personnel for LRA extensions and revocations as needed.

      

      

      

    Minimum Qualifications:  

    EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field. 

    EXPERIENCE: Entry level (no prior related work experience) 

    This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation. 

    LICENSURE: Agency Affiliated Counselor Registration.  

      

    Preferred Qualifications: 

    EXPERIENCE: Experienced (minimum 2 years of job-related experience) 

    Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment. 

    The capacity to work well with children and families.
