WeCare at Foltsbrook is actively seeking a motivated Director of Adult Day Care for our community in Herkimer, NY. 

Job Description:

·       Planning the adult day services program to meet individual needs of it registrants.

·       Liaison with community agencies to maintain referral sources bother to and from the program.

·       Oversee case management.

·       Facilitating advocacy efforts.

·       Coordinating the develop and ongoing review and monitoring of each registrant’s individual plan of care

·       Establish, maintain, and monitor the internal management systems to facilitate scheduling and coordination of services.

·       General supervision of program staff.

·       Monitor medication management.

·       Coordinate activities with other departments.

·       Involve registrants and family members in plan of case when possible.

·       Monitor, document, and recommend to your supervisor the equipment and supply needs of the program.

·       Monitor budget and financials for the department.

Shift:

Full Time

Candidate Qualifications:

·       Graduate of approved Registered Nurse (RN) Program.

·       Current license to practice as a Registered Nurse in the State of New York.

·       UAS certified

·       Experience supervising adult day care is preferred.

·       Minimum 3 years clinical experience
