Position Description: The Family Coordinator performs general functions as a secondary crisis worker in family/child advocacy working within community contracts. The Family Coordinator will provide primary and secondary crisis services for juveniles and their families, working to identify strengths, goals, while addressing potential barriers to success. The Family Coordinator will connect the family with behavioral and mental health resources to promote positive outcomes.
Family Coordinator also, performs function as a primary and secondary crisis services for children and families servicing through the course of the Boys Town service delivery model. Helping people where they are when they need help is foundation of Elevate and our service delivery .
The On The Way Home program provided by Boys Town will be guild and function of service programs while working with referral success and community partnerships to help children and families be successful and reduce the child from reoffending while in program care.
The Family Coordinator is responsible for ensuring appropriate delivery of services and coordination of care within health networks and with external entities to collaborate patient care effectively. The Family Coordinator provides excellent patient driven care and demonstrates superior positive regard toward all clients. The Family Coordinator will meet with clients in their homes, schools or other settings as required. The Family Coordinator will function in a multi-tasking, multiple priorities environment, while maintaining accuracy and attention to detail. The Family Coordinator is responsible for providing services to families and children, while providing timely updates of progress, barriers, and concerns to the referring agency and supervisor.
Essential Functions and Key Responsibilities
· Complete outreach to promote programs to include MCR, Juvenile Diversion Program within the 14-county program providing support and advocacy for juveniles
· Ensure contract deliverables are met and grant requirements are adhered to.
· Receive referrals for programs and ensure referrals are followed up on in a timely manner consistent with program requirements.
· Meet with families in their homes or other preferred location with appropriate regularity determined by program requirements and family needs.
· Complete required documentation and maintain accurate records within the Electronic Health Record.
· Maintain a caseload.
· Assign cases to other employees working cases within the scope of this program while providing supervision, support, and maintenance of all case records within the Electronic Health Record.
· Collaborate with supervisor and/ or Director of crisis services regarding intakes and case needs.
· Communicate with referring agency/worker, supervisor, and Elevate Team.
