    Reports To: Director of Nursing

    Summary

    He/she is responsible for the care of the residents in a designated area of the facility. He/she communicates with off-going and arriving shifts of personnel to assure the continuation of quality of care. Makes rounds, evaluating the physical, emotional and social needs of the resident making sure these are being met through the staff assigned to work with each group of residents. Assesses assigned staff’s ability to provide for each

    resident’s needs and provides intervention as indicated. Supervises the care provided by the Nursing staff including Nurses, and Nursing Assistants assigned to the unit, providing intervention, and education as indicated. Makes adjustments in assignments as indicated. Prepares and administers medications and

    treatments according to procedures. Consults with physicians, receives, records, and transcribes orders as

    required. Makes rounds with physicians when necessary. Reviews resident’s care plans and assures these plans are followed. Documents on records as per policy and procedures of the facility. Participates in the evaluation of the employee working under his/her supervision. Participates in staff meetings and in-services as required.

    Responsibilities

    1. Designated Infection Preventionist for the facility; responsible for overseeing the Infection

    Control Program for the facility including training, tracking, trending, monitoring, and preventions.

    2. Attends QAPI and QA meetings as scheduled.

    3. Participate and attend weekly High-Risk Meetings.

    4. Participate and attend daily QA morning meeting.

    5. Oversees Restorative Program including implementation and monitoring of residents on program.

    6. Performs designated trainings with staff to ensure competency according to standards of care.

    7. Daily oversight of data entry in the EHR to include; physician orders and documentation compliance.

    8. Oversight and management of medication rooms, supply rooms, medication cart, and other designated nursing areas.

    9. Oversee nursing schedule as designated by the Director of Nursing.

    10. Reviews communication systems to be knowledgeable of any unusual occurrences during the previous shifts.

    11. Assigns Nursing Assistants and other designated personnel assuring each resident’s needs will be met during the assigned shift.

    12. Makes rounds of the assigned unit frequently during the shift providing intervention as needed. During the entire shift, will be attune to needs of the unit while doing other nursing tasks.

    13. Prepares and administers medications as ordered. Reorders medications as needed in preparation for future shifts. Assesses and documents medication issues as per policy.

    14. Provides special procedures as ordered.

    15. Completes the treatments as ordered. Assesses and documents, new and old areas of treatment describing in detail the results of staff action and the results of treatment. Prepares a weekly skin report for the DON. Contacts the physician for any areas not responding positively and timely.

    16. Supervises the meal service for each resident on the unit. Works with the staff to assure each resident is provided adequate nutrition for each meal.

    17. Continually assesses and re assesses residents for risk of complications as a result of being in the facility. (I.e., skin breakdown, incontinence, contractures, reduced mobility, reduced involvement in bathing, dressing, or eating, weight loss, behaviors due to change in lifestyle, etc.).

    18. Continually observes progress and quality of work of the Nursing Assistants to assure that residents are turned, repositioned, toileted, walked, exercised, provided food and fluids, well groomed, involved in their specific restorative program, provides intervention as indicated, etc.

    ASSISTANT DIRECTOR OF NURSING JOB DESCRIPTION

    Also observes the resident's environment to assure a clean and safe environment is provided for each resident.

    19. Document on nurse’s notes as per policy. Assures there is accurate and timely documentation of any unusual observation, incident, or happening.

    20. Reviews information, reassess residents, interviews staff, reviews documentation of each shift and writes nurse progress note in the EHR.

    21. Admits residents as required.

    22. Assists in the ordering and reviewing of results of laboratory tests as indicated. Notifies physician of results. Oversees Lab Monitoring and overall lab program.

    23. Notifies the physician of unusual findings as indicated.

    24. Assists with auditing charts.

    25. Assists with answering and correcting consultant reports.

    26. Reviews, signs, and provides intervention as indicated for Nursing documentation.

    27. Assists in keeping other departments informed of resident’s change of condition as indicated.

    28. Performs any other tasks as assigned by the Director of Nursing.

    14. Must demonstrate ability to function as a cooperative team member with each member of the Nursing Department and other departments.

    15. Must demonstrate the ability to follow and supervise assigned staff to assure the facilities policies and procedures are followed and upheld.

    16. Must demonstrate the ability to manage and supervise the assigned unit in a productive, timely manner.

    Job Location

    Must work throughout the nursing areas of the facility. May require working irregular hours and may be exposed to communicable diseases and/or body fluids, medical preparations, toxic substances and occasionally ionizing radiation.

    Working Conditions

    Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses’ station, resident rooms, etc.).

    Moves intermittently during working hours.

    Is subject to frequent interruptions.

    Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions/circumstances.

    Is subject to hostile and emotionally upset residents, family members, personnel, visitors, etc.

    Communicates with the medical staff, nursing personnel, and other department directors.

    Works beyond normal working hours, and in other positions temporarily, when necessary.

    Is subject to callback during emergency conditions (i.e., severe weather, evacuation, post-disaster, etc.).

    Is involved in community/civic health matters/projects as appropriate.

    Attends and participates in continuing educational programs.

    Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.

    Education

    Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/RN program.

    Experience

    Must have, as a minimum, one (1) year of experience as a supervisor in a hospital, nursing care facility, or other related health care facility. Must have, as a minimum, of six (6) months of experience in rehabilitative and restorative nursing practices.

    Specific Requirements

    Must possess a current, unencumbered, active license to practice as an RN in this state.

    Must be able to read, write, speak, and understand the English language.

    Must demonstrate knowledgeable skills necessary to provide care appropriate to the age-related needs of the residents served.

    Must be a supportive team member, contribute to and be an example of team work and team concept.

    Must possess the ability to make independent decisions when circumstances warrant such actions.

    Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.

    Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.

    Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.

    May be necessary to assist in the evacuation of residents during emergency situations.
