    Identifies Medicare patients appropriate for Annual Wellness Visits (AWV). Uses critical thinking skills to
    plan and coordinate care.
    ESSENTIAL DUTIES AND RESPONSIBILITIES
    The essential functions include, but are not limited to the following:
     Review and update the patient’s medical history, including medications and chronic conditions.
     Perform comprehensive health assessment during AWVs for Medicare beneficiaries.
     Collaborate with healthcare providers to develop personalized care plans based on the assessment findings.
     Identify health risks and provide preventive care recommendations.
     Educate patients on preventive measures, health maintenance, and wellness strategies.
     Ensure that beneficiaries understand their healthcare plans and available resources.
     Encourage preventive screenings and vaccinations.
     Maintain accurate and detailed documentation of assessments.
     Report findings and updates to the healthcare team.
     Work closely with physicians, nurse practitioners, and other healthcare professionals to coordinate care.
     Participate in quality improvement initiative related to preventive care and wellness.
     Stay updated on Medicare guidelines and regulations.
    MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
     Must have a valid IN Registered Nurse license.
     Must have an associate degree in nursing.
     Previous experience in primary care, wellness, or community health.
     Familiarity with Medicare guidelines and policies.
     Effective communication and people skills to effectively interact with patients and healthcare professionals. 
     Ability to assess patients, identify health risks. 
     Proficient in maintaining accurate and detailed medical records.
     Ability to work collaboratively in a healthcare team environment.
     Willingness to stay updated on healthcare trends, Medicare policies, and best practices. 
     Initiative-taker who works independently; must be able to perform assignments with minimal or no supervision.
