The Director of Social Services is responsible for initiating and maintaining a comprehensive social services program on throughout the center. Social Services is responsible for identifying and providing for each resident’s social, emotional and psychological needs. The Director will assist in the development of a safe discharge plan. This position also requires the timely completion of MDS assessments, 48-hour meetings, initial, quarterly, and annual assessments.
Long-term care experience is preferred; however, we are willing to train the right candidate!
