CHCW has an opening for a Social Work Coordinator to provide case management to patients needing assistance in reducing barriers to care. This is a grant funded position. You will collaborate with the RN Care Coordinator, CWFM Residency Program and Care Teams to address social determinates of health and coordinate the integration of the social service function into patient care. Responsibilities include patient assessments, referrals to community resources, acting as a patient advocate, patient education, managing patient panels and performs standard patient care coordinator duties for the Residency Program and assists in the development of protocols for data tracking of health maintenance outcomes for the Resident panels.
Qualifications: Bachelor’s degree in Social Work, Social Sciences, or a related field is required; Master’s degree is preferred; equivalent combination of education and experience will be considered. 2 years recent case management experience is strongly preferred. Valid driver’s license, auto insurance and bilingual Spanish/English is required. Pre-employment drug screen required.
