    II. Job Summary:


    The Director of Health Programs leads the Home Care and Mental Health Programs to provide responsive services to people living with HIV or AIDS who have histories of trauma, unstable housing and co-occurring medical conditions. The Director does not need to be a licensed provider but does need excellent attention to detail and follow-through with high levels of initiative and commitment to achieving positive outcomes. The Director oversees the program staff and the health program policies and procedures consistent with all licensing requirements and annual agency goals and objectives. The Director is an active member of the leadership team helping to ensure services are delivered with compassion and consistency, and when applicable guides the growth of health services.


    The Director manages a team of Registered Nurses, Healthcare Liaisons, and a Mental Health Program Manager. The Director is responsible for ensuring staff are meeting all Minnesota Department of Health and Department of Human Services regulations and requirements for Clare Housing’s Comprehensive Homecare license. The Director also supports the Mental Health Program Manager in providing mental health related services.


    This is a full-time position at 40 hours per week. The salary range is $80,000 - $100,000 annually. Benefits include Medical, Dental, Vision, 4% match of 403B retirement fund, four weeks of Paid Time Off in year one, and more.


    Through the work of the Director of Health Programs people living with HIV at Clare Housing are able to stay in their homes, achieve health stability, and reach other significant quality of life goals.


    III. Core Job Functions:


    Program Supervision


    • Supervise the Home Care program to ensure compliance with all rules/regulations related to the MN Department of Health Comprehensive Homecare License.
    • Provide program leadership within the framework of Clare’s philosophy of Housing First, Harm Reduction strategies. Bring a trauma-informed lens to the work. Identify and pursue tangible opportunities for staff, residents, and the agency to deepen our impact with respect to racial equity and social justice.
    • Supervise Health Program staff including recruitment, training, scheduling, individual performance management, payroll review, and assisting staff during crises. This includes managing any contracted nurses, consultants, training vendors, or related contracts.
    • Lead the Health Program staff team in ongoing quality improvement, team development, problem solving and policy updates to adapt and improve services.
    • Establish annual program benchmarks and evaluate the success of Health Program services, leading staff and service improvements as needed.
    • In partnership with the Director of Supportive Housing and the HR Specialist, develop and ensure the delivery of trainings to support the professional growth of staff.
    • Proactively manage the Health Program departmental budgets and monthly outcomes, regularly working with staff to review service needs and rate setting.
    • Provide support and clear direction to staff when a client is in need of a higher or different level of care, in consultation with health and housing staff.


    Program Planning


    • Lead Health Program staff in investigating, planning, and pursuing any additional health services in consultation with the leadership team.
    • Provide strategies for program development and assist with related grant applications and the implementation of resulting grants or services.
    • Contribute to the overall organization strategy as a member of the leadership team, including participation in the development of agency-wide strategic plans, outcome goals, new service delivery models and other significant strategies and/or systems.
    • Create professional linkages and explore partnerships with local and regional HIV service organizations, health care programs, affordable housing programs and other community partners.


    IV. Position Requirements:


    Education and Experience:

    • Bachelor’s degree with minimum of five years of experience working in health programs and/or human services.
    • At least three years of experience in supervision, program management and compliance with external licensing requirements.
    • Experience in the areas of HIV/AIDS, mental and chemical health, Home Care services, and/or with people experiencing homelessness.
    • Degree in Nursing or a Masters Degree in Public Health, Social Work, Mental or Behavioral Health, or related field is preferred but not required.

    Knowledge, Skills and Abilities:

    • Perform at a high level independently, within the agency’s leadership team, and as the leader of an interdisciplinary program team.
    • Excellent attention to detail, time management, follow-through, and personal accountability for a range of program and staff related responsibilities.
    • Competence and commitment to working respectfully with marginalized communities including People of Color, LGBTQ individuals, people with histories of homelessness, people living with HIV, and people with mental illness, substance use disorders, complex trauma and co-occurring health conditions.
    • Direct, lead, and provide accountability within complex programs while motivating program staff.
    • Effectively represent Clare Housing programs to community officials, funders, and the general public.
    • Effective ongoing development and management of program budgets.
    • Maintenance of professional boundaries, self-care practices, and the emotional stability needed to cope with the mental and emotional stress of the position.
    • An understanding and commitment to implementing Housing First and Harm Reduction philosophies and advancing Racial Equity.
    • Knowledge of local, state and federal regulations pertaining to Homecare Licensure, HIPAA privacy laws, HIV/AIDS disease impacts and treatment options, and mental health/substance use service opportunities (or the ability to gain this knowledge quickly and accurately).
    • Proficiency in computer applications such as Microsoft Office and electronic health record systems.
    • Access to reliable vehicle and ability to maintain good driving record and insurance levels, as directed by the agency, to fulfill job duties that require travel in the community.


    V. Working Conditions and Environment:


    This position is located in a supportive housing building for adults living with HIV/AIDS and other disabilities. Many of the residents are formerly homeless individuals. The building is accessible and smoke free. This position works during normal business hours which may include evening and weekend hours as needed or as requested. This position is expected to work from Clare Housing sites 60% of the time at a minimum, though may be 100% in person as needed or desired.


    The work of this position requires regular travel to multiple program locations locally, and on occasion, to travel within the state and nationally. The person in this position must be able to make public presentations and to present effective training for staff.
