JOB SUMMARY:
Provides medical direction for patient care delivered by the Hospice team. Provides direct care and consultation upon request.
ESSENTIAL FUNCTIONS:
Serves as a member of the interdisciplinary team (IDT).
Provides medical direction and expertise to the IDT team. Participates in admission, certification, recertification and discharge decisions.
Participates in the development of plans of care that are medically appropriate and that meet accepted palliative care and Hospice & Community Care standards and guidelines.
Consults and maintains communication with the patient's attending physician as needed in the coordination of care and service.
Monitors medical plans of care to assure consistency with Hospice & Community Care palliative medicine policies and guidelines.
Provides direct patient care medical evaluations when necessary and requested as part of the plan of care, and documents visits and services according to Medicare standards.
Participates in decisions related to admissions and services in the inpatient center: Provides timely medical assessment for new admissions.
Assists physicians in the provision of patient visitation and medical management when requested by primary/attending physicians.
Participates in the provision of 24 hour per day, 7 day per week physician services.
Serves as a representative of Hospice & Community Care to the community-at-large.
Serves on the Medical Advisory Committee.
QUALIFICATIONS:
Doctor of medicine or osteopathy degree from an accredited medical school
Licensed to practice medicine in Pennsylvania.
Board certified in a medical specialty (medicine, oncology, family practice).
Hospice and Palliative Care certification preferred.
Has demonstrated interest, knowledge, and skills in hospice care.
Pediatric experience a plus.
Member in good standing on the medical staff(s) of accredited hospitals in the Hospice & Community Care service area.
Able to effectively communicate verbally and in writing, and to motivate, lead, and provide specialized medical direction, particularly in a team setting and also in physician-to-physician relationships.
Must have working knowledge of computer based programs and have the ability to learn and navigate our computer system.
Carries medical malpractice and liability insurance in amounts satisfactory to Hospice & Community Care.
