As a Social Work Case Manager, your job is equal parts heart and mind, and you will play a vital role in the care of our greatest generation, our seniors, as you help to provide comfort, security, and peace of mind for, as you aid our treasured residents and their families in making Heritage their home.
Our ideal candidate will have an Associates Degree or Bachelor degree in Sociology, Human Services, Psychology, or a closely related field. Additional education or experience in long-term care is preferred, but not required.
