The Hospital Case Manager will play a vital role in ensuring that patients have a smooth transition from an inpatient acute care setting to a post acute rehabilitation program or skilled nursing facility. The Inpatient Case Manager will be responsible for organizing complex care, assessing patient's health insurance plans and providing necessary resources for patients being discharged. The Case Manager will act as an advocate for their patients and go above and beyond to help accurately assess their individual needs.
