_ Job Description_
Provides services to individuals with psychiatric disabilities that focus on maintaining individuals overall independence in the least restrictive environment possible while teaching life skills such as daily living, budgeting, medication management training and other skills like taking transportation in order to live the most fulfilling life possible. Locates needed services/agencies and makes referrals and arrangements for treatment and support services related to the Individualized Recovery Plan. Ensures the individual gains access to needed services by assisting the individual as they move between and among services and supports. Actively assists the individual to acquire needed resources including income, entitlement benefits, housing, transportation, etc. Implements and organizes the delivery of specific social services within the community. Maintains contact with other social service agencies and health care providers involved with individuals’ overall progress and recovery. Maintains service productivity. Provide 24/7 on-call phone access for crises as scheduled. Must be dependable and flexible. Must work as scheduled.
Minimum Qualifications
High School Diploma and/or Certified Peer Specialist. At least two years of experience in behavioral health case management. Must have a valid driver’s license with a good driving record. Proficiency in MS Word, Excel, Outlook and Power Point software.
Preferred Qualifications
Bachelor’s Degree with 2-3 years’ experience. Prior experience in documentation through electronic medical records.
