We are a Support Services Brokerage and a nonprofit 501(c)3 agency that provides case management to adults with intellectual and developmental disabilities living in the communities of Coos, Curry, Douglas, and Klamath counties.
Position Available
We are seeking a full-time Personal Agent (Case Manager) to work in our Coos Bay office.
This is a 40-hour-per-week position, to be worked within the core organizational hours of 8:00am – 4:30pm, Monday through Friday, with periodic flexibility to accommodate customer or organizational needs as required.
This is not a direct support position, but rather a comprehensive case management position.
As a Personal Agent, you will help customers (the individuals we serve) and their families navigate services. Through a person-centered process, you will meet with customers and their chosen participants to plan, develop, and implement individualized support plans (ISPs) based on their needs, goals, and dreams. You will advocate with and on behalf of your customers to obtain needed services, resources, exceptions, etc. You will provide choice advising and facilitate connection with providers. You will also monitor the quality of services provided and attend to changes in need and emergent issues as they arise.

Compensation & Benefits
