Behavioral Health Case Management Manager jobs in Galveston, TX

Minimum Qualifications: 
Bachelor's degree in Psychology, Social Work or Mental Health / Behavioral Science. Criminal Justice degree considered with 15 hours of coursework in psychology, social work and/or counseling with 2 years of MH case management experience.

Job Summary: 
This position is responsible for providing case management, social and habilitative services, and counseling to patients. Works under the supervision of a Psychologist, Mental Health Manager or Program Director.

Job Duties:

• Collaborate with program clinicians to coordinate ongoing care for TCHATT patients
• Act as liaison between patients and clinicians
• Disposition TCHATT cases upon discharge from services 
• Develop and coordinate care plans for patients
• Assist with development and maintenance of the mental health resource database
• Demonstrate competency and skill in communicating with patients, families, and school personnel
• Participate in team meetings and provide status updates to team members and leadership
• Participate in conferences, workshops, and trainings related to your role
• Assist with maintaining project data and required reports
• Other duties as assigned

Preferred Qualifications:

• 1 year experience working with families.
• Must have proficient computer skills.
