• Monday to Friday; 10:00 AM – 7:00 PM

PAY RANGE:

• $20.00 - $21.00 per hr.

Job Summary:

The Behavioral Health Case Manager (BHCM) is responsible for identifying patients with mental health needs, supporting them through providing mental health skills interventions, and monitoring their progress in the integrated behavioral health program. The BHCM provides evidence-based services as well as works with other members in the integrated health care environment which includes the primary care physicians, other behavioral health specialist, and consulting psychiatrist to provide a collaborative system of care. The BHCM maintains an organized tool used to track mental health service needs and responses to treatment. The BHCM also provides patients, family members, and community psychoeducation about mental health and available mental health resources. 

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

• Understands the importance of patient confidentiality and maintains records in a manner as such to comply with HIPPA guidelines on patient privacy.
• Accepts referrals from primary care providers for patients who have accepted treatment under the integrated behavioral health model.
• Screens patients who have been referred to behavioral health services and educates them on the behavioral health program.
• Initiates contact with each new patient within one week of referral and maintains 4-weeks of brief skill intervention for mental health needs while client waits to be transferred for therapy.
• Collaborates with patients to monitor and track patient desires for mental health goals. 
• Works in collaboration with primary care team to ensure patient's mental health needs are being met by providing support to patient as needed.
• Uses phone, in-person, or video methods to communicate and follow up with patient for their skills intervention appointments and other appointments relevant to care for mental health. 
• Documents patient progress in EHR system (call attempts, sessions, and communication) as well utilizes tools used to measure patient progress and retention.
• Provides patient assistance when called on by provider to assess initial symptoms that may require behavioral health program referral and provide patient with follow-up to screen symptoms and progress. 
• Support psychotropic medication progress by educating clients on importance of medication consistency, side effects, and role medication plays in effectiveness of treatment. 
• Participates in and engages with behavioral health team on a regular basis to address and track progress of patients that may need follow-up or referrals in the community. 
• Advocate or facilitate changes with behavioral health team or community agencies to increase patient's quality of care.
• Provide psychoeducational groups/classes to patients at our clinic and/or in the community.
• Maintain all professional relationships with mental health, substance use, and other community providers to foster coordination of services. 
• Comply with all company policies, procedures and regulatory requirements.
• Performs all other duties as assigned by management.

Qualifications

SKILLS AND EXPERIENCE:

• A regard for high ethical standards and mental health patient care.

• Well organized, punctual, and able to communicate with others well.

•  Able to work on a team well and also work well independently with specific caseload.

• Advocate for client's mental health needs, actively respond to inquiries/complaints, and maintain patient confidentiality.

• Ability to multi-task and be innovative in carrying out work responsibilities.

• Comfort speaking in public spaces to educate the community about services.

EDUCATION/LICENSURE/CERTIFICATION QUALIFICATIONS:

• Bachelor's Degree in Social Work, Counseling, Psychology or Related Field from an accredited university
• *Bilingual in Spanish preferred; not required
