Job Summary:
The Behavioral Health Case Manager (BHCM) is responsible for identifying patients with mental health needs, supporting them through providing mental health skills interventions, and monitoring their progress in the integrated behavioral health program. The BHCM provides evidence-based services as well as works with other members in the integrated health care environment which includes the primary care physicians, other behavioral health specialist, and consulting psychiatrist to provide a collaborative system of care. The BHCM maintains an organized tool used to track mental health service needs and responses to treatment. The BHCM also provides patients, family members, and community psychoeducation about mental health and available mental health resources.

Comfort speaking in public spaces to educate the community about services.
