Onyx Behavioral Health is an AHCA licensed primary mental health facility. We are currently seeking a dedicated Case Manager to join our team. The ideal candidate will provide case management services (e.g., information, plan referral, and linkage). CM will facilitate patient care by identifying and assessing patient needs and evaluate, coordinate, and oversee necessary services and/or treatment is provided. Also, provides support and assistance to clinical department in various activities. Works with and in consultation with licensed clinical and medical staff as part of multi-disciplinary team.
Responsibilities:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide crisis intervention and emotional support to clients in difficult situations
- Collaborate with other professionals and agencies to coordinate services for clients
- Monitor and evaluate clients' progress towards their goals
- Advocate for clients' rights and ensure they have access to appropriate services
- Maintain accurate and up-to-date case notes and documentation
Skills:
- Experience in behavioral health and crisis intervention
- Ability to supervise and mentor individuals
- Knowledge of child welfare and workforce development programs
- Familiarity with addiction counseling and Medicare regulations
- Proficiency in childcare services and Section 8 housing assistance
