The Crisis Case Manager (CCM) is responsible for responding to and/or coordinating a response to all crises during the shifts associated with position. Full time position. Day Shift: Wed. Thursday, Fri. Sat 8 am - 7 pm, Wed. or Evening Shift Thursday, Friday, Sat. 6 pm - 8:30 am. All Staff Meeting at 8 am. The CCM provides crisis follow-up services until the crisis is resolved or the person is enrolled into the Mental Health system. In addition, provide hospital liaison activities for all hospitalized persons from Pacific County; responsible for monitoring least restrictive alternatives (LRAs) and work with hospitals and providing follow- up services to clients being released back to the community or inpatient mental health facilities.
