Position Overview:
The Outpatient Case Management Nurse position will coordinate the interdisciplinary plan of care for the non-hospitalized population (multi-visit patients, TCM, addressing SDOH) for SRHS service area. Acts as a liaison between the patient/family and healthcare personnel to ensure all necessary care is provided and promotes quality, cost effective care and patient outcomes. Must be able to rely on experience and judgment to plan and accomplish goals and perform a variety of tasks with a certain degree of creativity and latitude.
