The ideal candidate will have the following responsibilities:
Provide community based, mobile mental health treatment to individuals diagnosed with a Serious Mental Illness (SMI) and a history of persistent service needs. Work as part of a multi-disciplinary team to develop a comprehensive, integrated set of medical and psychosocial services, aimed at maximizing independence and advocating for the rights of persons served. Perform the duties of a Case Manager (linking, coordinating, monitoring, advocating).
To be successful, this candidate must have
1. Bachelor's degree required in Social Work, Psychology, or other related Human Service field.
2. Must meet standards as a Qualified Mental Health Professional and Qualified Intellectual Disabilities Professional at the end of one year of employment in the position.
3. Knowledge and/or experience working with persons diagnosed with substance use disorders helpful. Knowledge and/or experience facilitating groups.
4. Demonstrated ability to provide customer service, write reports, clinical correspondence and procedure manuals; effectively present information and respond to questions from groups of Board members, manager, persons served and the general public.
5. Appointment is subject to driver’s license and criminal background check. Must be 18 years of age, possess a valid driver's license, and provide own transportation.
