    Position Summary:

    Case Managers facilitate the development of discharge plans for all patients starting from the moment of admission. They meet 1:1 with patients to collaborate with their treatment and discharge planning; consult with families on the emotional, social, and financial aspects of the patient’s life; and assist with family meetings with patients (when warranted). They also play a valuable role in multidisciplinary team meetings, patient care conferences, and treatment planning. Case Managers comply with required standards for medical/legal reporting situations (e.g., child abuse, dependent adult abuse, duty to warn, etc.). They may perform other duties as assigned.
    Qualifications:
    Qualifications

    Education:
    Required – Bachelor's degree from an accredited college or university with mental health field experience

    Preferred – Inpatient mental health case management experience, experience referring to community partners, understanding of involuntary treatment system in WA

    Additional requirements:
    • Agency Affiliated Counselor credential (must bring paperwork to orientation for completion)
    • Required trainings provided by Fairfax – CPR, First Aid, Handle with Care, and Verbal De-escalation

    7. Ability to communicate effectively with patients and co-workers
    8. Ability to adhere to safety policies and procedures
    9. Ability to use good judgement and to maintain confidentiality of information
    10. Ability to write neatly and legibly
    11. Ability to perform accurately in an environment with high levels of stimuli
    12. Ability to work as a team player
    13. Ability to demonstrate tact, resourcefulness, patience, and dedication
    14. Ability to accept direction and to adhere to policies and procedures
    15. Ability to work in a fast-paced environment with competing priorities
    16. Ability to perform repetitive tasks and to work at a rapid pace
    17. Ability to react calmly and effectively in emergency situations

    Physical, mental, and special demands:
    1. Hand and finger dexterity for the use of computer equipment
    2. Ability to sit for extended periods of time, up to eight (8) hours at a time
    3. Ability to stand and walk for up to six (6) hours at a time
    4. Ability to move flexibly for filing and retrieving of medical records
    5. Ability to lift, bend, stoop, twist, pull, and push is required
    6. Visual acuity is required for accurate communication of departmental and medical information
    7. Ability to frequently lift and carry up to 10 pounds (e.g., charts, records)
    8. Ability to work in an environment in which the noise level is frequently high; where chemicals are frequently used for cleaning; where mechanical and electrical hazards may occasionally be present; and where dust, mist, and steam are frequently generated in housekeeping tasks
    9. Ability to respond as trained when exposed to blood and bodily fluids
    10. Ability to participate in the management of acutely agitated patients consistent with Handle with Care hospital procedures
