Job Summary:
The Case Manager will assist clients in a residential environment with accessing a whole spectrum of services from insurance coverage, housing, to connecting people to recovery programs and other resources. The incumbent will be responsible for providing education to all clients surrounding addictive disorders, mental health conditions, and life skills to enhance their long term recovery plan.
Qualifications:
The position requires a two-year associate degree in human services or commensurate experience; wellness coach, peer support specialist; chemical dependency professional candidate. A Bachelor's degree in human services is preferred.
Essential Duties and Responsibilities Include the Following:
1. Coordinates services for clients participating in intensive inpatient residential treatment.
2. Assist clients when problems develop and refer identified concerns to local community support systems.
3. Develops current needs assessments for all clients participating in the residential program.
4. Implements and participants in programs for clients to include social skills activities, community resource, accessing self-help skills, parent education, facilitating lectures.
5. Assist clients applying for funding sources such as DSHS or SSA assistance.
6. Documents client progress and adherence to chronological notes, client programing plans and reviews, vocational goals, educational goals, substance abuse goals, employment goals and or any other services needed for clients.
7. Consults or represents agency with courts and other agencies as assigned by supervisor.
8. Will assist in assuring program compliance with administrative directives as well as state and federal regulations.
9. Work within a multi-disciplinary team onsite to assist in the transition to the client's next appropriate level of care.
10.Mediate client disputes to ensure that all conflict is handled in a solution focused modality.
11.Other duties as assigned by administrative and/or clinical supervisor.
Physical Demands:
Must be physically able to perform duties of the position. Must be able to use computer keyboard and view monitor for long periods of time.
Work Environment:
Moderate noise (residential treatment facility/offices with computers, printers, and light traffic.)
