Behavioral Health Director oversees the study and treatment of patients with emotional, mental, or psychological disorders. Develops, implements, and updates strategic initiatives, programs, and services. Being a Behavioral Health Director manages and directs the operations of behavioral health services. Uses knowledge of behavioral health research and trends to ensure the department's practices are up-to-date and effective. Additionally, Behavioral Health Director establishes training programs and supports and supervises staff. Ensures all behavioral health operations comply with clinical guidelines and regulations. Requires an advanced degree in psychology or related field. Typically reports to top management. The Behavioral Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Behavioral Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     Broadstep Behavioral Health Inc. is seeking a Care Manager to serve as the leader of our treatment team that provides direct clinical care to adults and children in our community based and in-home care programs in North Carolina. This position works closely with other clinical members of a defined team and serves an integral role in the treatment of patients in the behavioral health continuum.

    Schedule & Location: Full time, M-F, 8am-5pm serving the greater Wilmington, NC area.

     Job Type:
    (Community Based/Onsite)

    Job Duties:
    • Develop care management comprehensive assessments and care plans/individual support plans with beneficiaries
    • Coordinate/refer/monitor all services (medical, pharmacy, behavioral health, waiver services, food, housing, transportation, community resource supports)
    • Support beneficiaries in a crisis (with planning supports)
    • Arrange for annual physicals
    • Innovations and TBI waiver care coordination (if applicable)
    • Convene and consult with a multidisciplinary care team
    • Provide management for beneficiaries with chronic, high-risk, high-cost care management needs
    • Help with medication monitoring
    • Address unmet health-related resource needs
    • Educate on chronic health conditions and support self-health management (eating healthier; helping you join a diabetes prevention program)
    • Monitor Hospital Admission Discharge and Transfer (ADT) alerts and ensure beneficiaries with any admissions, discharges or transfers are followed
    • Support transitions out of hospitals and nursing facilities
    Required Education/Experience:
    • A bachelor's degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area, or licensure as a registered nurse (RN)
    AND
    • Two years of experience working directly with individuals with behavioral health conditions (if serving members with behavioral health needs) or with I/DD or a TBI condition(s) and
    • For Care managers serving members with LTSS needs: two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience, in addition to the requirements cited above.
