    Job Summary:
    (Golden Modesto Care Center ) in (Modesto, Ca) is seeking an Admissions Coordinator to manage the facilities inquiry and admission process and provide customers with community-related information via facility tours, personal visits/assessments, conversations and follow-up. The Admissions Coordinator also implements effective sales and marketing programs for the community

    Essential Functions:
    • Demonstrates exceptional customer service skills to all customers.
    • Responsible for taking referral information from telephone and walk-in inquiries, obtaining necessary personal, clinical, and financial information.
    • Works with Interdisciplinary Team to meet and exceed conversion ratios and census goals.
    • Works with the Business Office Manager to ensure prompt coordination for financial verification and ensures financial verification and forms are accurate and complete.
    • Coordinates the pre-admission process and acts as liaison between the customer and the community.
    • Coordinates the pre-admission process to include clinical information to identify care/service needs, pharmaceutical and DME requirements, and financial information.
    • Escalates and obtains approvals from clinical team to ensure consistent pre-admission process.
    • Completes Admissions Packet with patient and/or families, patient representatives (Guardian, POA, DPOA) within 72 hours of admission.
    • Participates, as needed, in daily stand up meeting to update facility marketing/admission team on referrals, pending admissions, and scheduled tours.
    • Conducts outreach to referral sources, including, but not limited to: hospital case managers, home health agencies, independent and assisted living community, insurance case managers, physicians and hospice agencies, as assigned.
    • Creates and implements monthly marketing plan for skilled nursing center, with direction and support of Administrator and Director of Sales and Marketing
    • Attends and participates in in-service training, performance improvement (“PI”) committees and other meetings as scheduled and directed.
    • Knows Resident Rights. Helps the residents/patients exercise and/or protect their rights.
    • Reports resident/patient complaints to management.
    • Maintains confidentiality of resident/patient information.
    • Follows and adheres to policies and procedures implementing HIPAA requirements for the privacy and security of protected health information.
    • Other duties as assigned.
    Education, Licensure, & Experience Requirements:
    • High School diploma or equivalent
    • Two years of experience interacting and working with hospital-based clinicians (physicians, nurses, discharge planners and social workers).
    • Two years’ experience in an outside sales and marketing role, with demonstrated experience in making sales calls to healthcare professionals.
    • Possesses a valid driver’s license and has access to a vehicle.
    • Computer proficient.
    • Ability to work independently with minimum supervision, problem solving, conflict management and budgeting.
    Knowledge, Skills & Abilities:
    • Ability to follow directions and work independently with minimal supervision. Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff.
