Position Summary
Under the direction of the Group Home Manager, the Healthcare Coordinator will coordinate and manage all medical appointments for the residents at Curry Villa. This includes but is not limited to scheduling and attending all medical appointments, scheduling future appointments, and communicating with the Group Home Manager and Circle of Supports any medical concerns and expected follow through. Directly reports to Group Home Manager.

Requirements
Essential Responsibilities
- Follow Parc Center for Disabilities’ philosophy, policies, and procedures
- Responsible for scheduling and attending all residents’ medical appointments
- Responsible for following up on medical concerns in a timely manner
- Assist with any emergency room visits
- Responsible for maintaining and providing current and appropriate medical paperwork at time of medical visit
- Ensure that all forms are filled out correctly by the MD or Nurse Practitioner prior to leaving the office
- Complete MAR Audits and inventory checks on a weekly basis
- Ensure all prescribed medications are in stock and necessary medication administration supplies are available
- Coordinate the delivery and pick up of all MARs, prescriptions and orders from various PCPs and Pharmacy
- Responsible for reordering medications that are running low
- Provide continuous training for all team members on medical and medication policies and procedures
- Ensure the on-going documentation of medical concerns and changes as needed
- Prepare, maintain, and update resident medical files monthly
- Train new team members on Curry Villa’s Medical Policies and Procedures in addition to MAR documentation
- Prepare quarterly/annual medical summaries for each resident
- Maintain Parc Center for Disabilities’ driver’s certification
Administration
- Report to the Group Home Manager satisfaction data, client demographics, personal outcomes, incident reports and all data required for Outcome Measurement and other Accountability Reports
- Coordinate additional transportation needs with Parc’s transportation department
- Order and maintain necessary supplies along with routine monitoring of needed equipment
- With the guidance of Group Home Manager, oversee compliance with monthly and annual state licensing and Governing Agency monitoring
- Complete client intakes and act as a liaison with parents, guardians, and Support Coordinators
- Ensure the smooth integration of day-to-day operations, including medication administration
- Attend Circle of Support meetings for all residents
- Fill in as a Direct Service Provider when needed
Quality Assurance:
- Ensure that quarterly reviews of maintained medical and working files are completed, and appropriate guidelines are established to bring files into compliance
- Coordinate with other departments (Adult Day Training, and Transportation) as necessary
Education, Training and Experience Requirements
- Bachelor’s Degree in a related field required
- Must have at least four years’ experience in the field of Intellectual and Developmental Disabilities or a related field
- Direct work experience, in lieu of education requirements, may be considered
- Valid Florida Drivers’ License with acceptable driving record
Knowledge, Skills & Abilities
- A heart for people first and foremost.
- Have a collaborative, agile approach, with a growth mindset. The ability to quickly pivot is important.
- Able to pass all required trainings, including but not limited to Professional Crisis Management and Medication Administration training and validation, CPR/First Aid
- Strong analytical, written, and verbal communication, employee interaction, decision-making, organization, and time management skills
- Proficient in the Microsoft Office Suite, Therap, and other electronic tools like Zoom and Paylocity.
- Able to safely lift up to 50 pounds
- Able to stand for extended periods of time
- Be physically, visually, and auditorily aware of the cognitive, social, and physical needs of our residents
- Communicate effectively in English both verbally and in writing
Schedule
- Normal business hours are Monday through Friday, 8:00 AM to 5:00 PM. Your schedule may vary depending on event schedules, and other factors.
- This is an on-site role.
