Bereavement Coordinator plans and provides bereavement counseling and support to survivors. Assesses individual needs and coordinates with multi-disciplinary teams to develop and deliver a holistic support plan. Being a Bereavement Coordinator guides survivors to appropriate bereavement resources. Facilitates grief support group sessions. Additionally, Bereavement Coordinator requires a bachelor's degree in social work. May require the applicable State License to Practice Clinical Social Work. Typically reports to a manager. The Bereavement Coordinator occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function. Following the interdisciplinary team plan of care, the Chaplain organizes and provides counseling and guidance mindful of the patient’s, family members, and employee’s right to confidentiality. Participating actively in meeting the goals and objectives of the hospice organization, the Chaplain collaborates and communicates with the Case Manager and members of the interdisciplinary team. The Chaplain performs and documents care and counseling adhering to the hospice, regulatory body, and chaplaincy standards
