Bereavement Coordinator plans and provides bereavement counseling and support to survivors. Assesses individual needs and coordinates with multi-disciplinary teams to develop and deliver a holistic support plan. Being a Bereavement Coordinator guides survivors to appropriate bereavement resources. Facilitates grief support group sessions. Additionally, Bereavement Coordinator requires a bachelor's degree in social work. May require the applicable State License to Practice Clinical Social Work. Typically reports to a manager. The Bereavement Coordinator occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function. The Spiritual Coordinator will directly provide and coordinate spiritual care services and counseling to patients and families in accordance with the Hospice plan of care, current and accepted standards of practice, and Medicare, Medicare and third party payer requirements. Works collaboratively with other care team members, the physicians and the patient/family to achieve optima patient outcomes. Has access to confidential employer, patient and financial information. Provides direct patient care to al ages through the continuum of care.
Required Educational Degree: Associate's degree in ministry
Required Certifications: CPR Certification required
Required Experience: 2 years experience in active ministry
