Bereavement Coordinator jobs in High Point, NC

Bereavement Coordinator plans and provides bereavement counseling and support to survivors. Assesses individual needs and coordinates with multi-disciplinary teams to develop and deliver a holistic support plan. Being a Bereavement Coordinator guides survivors to appropriate bereavement resources. Facilitates grief support group sessions. Additionally, Bereavement Coordinator requires a bachelor's degree in social work. May require the applicable State License to Practice Clinical Social Work. Typically reports to a manager. The Bereavement Coordinator occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function.   • Essential Duties
    • Assesses client’s coping ability including a review of physical and psychological status, anticipated socioeconomic changes, and the availability of a positive support system. Evaluates whether the client is experiencing normal or complicated grief.
    • Develops and provides timely implementation of a plan of care according to the assessed needs and expressed wishes of the client. The plan of care will promote successful coping skills and will be consistent with all the appropriate rules, regulations and standards of care.
    • Provides bereavement counseling and crisis intervention as necessary to develop client’s coping skills and to provide emotional support.
    • Reassesses client’s progress at each contact and amends plan of care as necessary.
    • Clearly documents all interactions according to organizational policy and procedures for timeliness, content, credentialing and records management.
    • Plans and facilitates support groups and social support opportunities at least on a quarterly basis.
    • Plan and implement community grief education and supportive programming throughout the year. Provides programs in the organization and community related to grief, successful strategies for coping with loss and all Hospice of the Piedmont services.
    • Makes follow-up phone calls in order to check on client’s progress and complete bereavement assessments.
    Requirements
    • Master’s degree in social work, psychology, or counseling
    • Two years related experience preferred
    • Licensed by the NC Social Work Licensure Board as a Licensed Clinical Social Worker, licensed by the NC Board of Licensed Clinical Mental Health Counselors as a Licensed Clinical Mental Health Counselor, or eligible for licensure as LCSW or LCMHC in the state of NC is preferred. Other related counseling licensures may be considered.
    • Valid NC driver’s license, reliable independent transportation, and current automobile liability insurance at limits required by the agency
