Bereavement Coordinator jobs in Plano, TX

Bereavement Coordinator plans and provides bereavement counseling and support to survivors. Assesses individual needs and coordinates with multi-disciplinary teams to develop and deliver a holistic support plan. Being a Bereavement Coordinator guides survivors to appropriate bereavement resources. Facilitates grief support group sessions. Additionally, Bereavement Coordinator requires a bachelor's degree in social work. May require the applicable State License to Practice Clinical Social Work. Typically reports to a manager. The Bereavement Coordinator occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function.     In this role that reports to our Director of Clinical Services, you will provide coordinated care for hospice families of all age groups and provide counseling and referrals to meet the needs of the hospice patient and their family members. 


    Job Qualifications

    • High School Graduate, Bachelors in Human Services preferred. 
    • Current Drivers License 
    • Experience in bereavement and grief counseling. 
    • Working knowledge of community resources
    • Good interpersonal skills. 
    • Reliable transportation with valid auto liability insurance


    Essential Functions

    • Assess hospice family to identify bereavement needs as evidenced by documentation, and community resource referrals. 
    • Use effective interpersonal relations and communication skills. 
    • Meet mandatory continuing education requirements of the agency. 
    • Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards. 
    • Promote our philosophy and administrative policies to ensure quality of care. 
    • Collaborate with the interdisciplinary team to promote coordination of bereavement care.
    • Provides continued bereavement support to hospice families as evidenced by documentation
