Bereavement Coordinator plans and provides bereavement counseling and support to survivors. Assesses individual needs and coordinates with multi-disciplinary teams to develop and deliver a holistic support plan. Being a Bereavement Coordinator guides survivors to appropriate bereavement resources. Facilitates grief support group sessions. Additionally, Bereavement Coordinator requires a bachelor's degree in social work. May require the applicable State License to Practice Clinical Social Work. Typically reports to a manager. The Bereavement Coordinator occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function.     This position is part of the hospice bereavement program. It participates as part of the hospice interdisciplinary team, provides bereavement outreach/engagement calls, in-person and virtual group and individual counseling to hospice and community bereaved, facilitates grief presentations and workshops, and participates in/contributes to all bereavement events such as Light Up a Life and Camp Erin, among other responsibilities. It is primarily day shift with some intermittent variability-for example, facilitating an early evening grief group, weekend or evening trainings related to Camp Erin, Camp Erin itself, etc. The position cross-supports across team service areas as needed though will primarily be assigned responsibility in West counties.

    Required qualifications:

    • Master's Degree Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE).

    • Upon hire: Oregon Clinical Social Worker License Or

    • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.

    • 2 years Relevant Social Work experience. May include CSWA experience.

    • 2 years Post Masters experience in Hospice, Palliative Care, Bereavement, Mental Health, or Health Care.

    • 2 years Post Masters experience in Hospice, Palliative Care, Bereavement, Mental Health, or Health Care working with children and adolescents required.

    Preferred qualifications:

    • 2 years clinical work experience in Hospice or Palliative Care.

    • 3 years social work experience.

    Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
