Bereavement Coordinator plans and provides bereavement counseling and support to survivors. Assesses individual needs and coordinates with multi-disciplinary teams to develop and deliver a holistic support plan. Being a Bereavement Coordinator guides survivors to appropriate bereavement resources. Facilitates grief support group sessions. Additionally, Bereavement Coordinator requires a bachelor's degree in social work. May require the applicable State License to Practice Clinical Social Work. Typically reports to a manager. The Bereavement Coordinator occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function.     Salary or hourly rate listed is the base for this role; increase commensurate with experience.

    Position Overview – Bereavement Coordinator:

    • Represent Empyrean Hospice with the utmost professionalism and compassion

    • Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers

    • Plans for Bereavement Program development, expansion, and refinement

    • Provides bereavement support to hospice personnel coping with work related grief through one-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources

    • Provides bereavement information and referral services to the community; maintains community resource list

    • Ensure standards of ethical business and clinical practice are prioritized

    Position Qualifications

    • Professionalism, compassion, empathy, and a desire to help others

    • Self-directing with the ability to work with little direct supervision, yet also as member of a complete interdisciplinary care team

    • Bachelor’s degree or equivalent theological degree from college or university

    • Experience with families/caregivers and patients through death/dying issues

    OR

    Master's degree in social work with qualifications of experienced clinical grief counseling.

    • Minimum of one year experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying

    • Knowledge and commitment to hospice philosophy

    • Understanding of grief response and experience with counseling individuals and/or in groups regarding loss

    • Demonstrate good verbal and written communication, and organization skills

    • Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
