Essential Job Functions Coordinates the assessment and delivery of grief counseling needs and services. Provides bereavement support to hospice personnel coping with work related grief through one-to-one counseling. Assists as needed in evaluating and planning services to meet hospice personnel support needs. Participates in hospice activities such as in service education, hospice personnel meetings, and relevant committees. Actively participates in quality assessment performance improvement teams and activities. Determines the level of care and support of bereaving families and significant others following the death of a hospice patient. Reflects the needs of the family in the bereavement care plan and updates and revisions are conducted on an ongoing basis. Implements the Plan of Care provisions related to the bereavement needs of families & significant others. Qualifications: Bachelor’s degree or equivalent theological degree from college or university OR, Master’s degree in Social Work with qualifications of experienced clinical grief counseling. Experience with families/caregivers and patients through death & dying issues. Ability to work as a team member. Knowledge and commitment to hospice philosophy. Good verbal and written communication, and organization skills. Understanding of grief response and experience with counseling.
