First Choice In-Home Care is seeking a full-time Social Services Case Manager to manage a diverse Client caseload consisting of vulnerable and/or at-risk minors, adults and seniors with a primary focus on consolidating the service recipients' care team through the addition of qualified care providers. Our aim is to enable our clients to continue living in the comfort and safety of their home setting, versus becoming homeless or being transitioned to an institutional living environment, through establishing and then sustaining their ongoing, in-home care team.

First Choice is licensed by the Washington State Department of Health (DOH) and contracted with the Washington State Department of Social and Health Services (DSHS) to provide personal care services to our population of developmentally disabled children & adults, individuals with mental health disorders, veterans and seniors. We provide services to both State funded (Medicaid) and Private clients living in King, Pierce and Snohomish counties. We are an established and still growing Agency managing the care of over 2,600 vulnerable community members and are now hiring qualified, passionate and motivated staff members into our King and Pierce County Case Management Departments.

The position is full-time, Monday-Friday 8:30am-5pm
