We operate a shelter that supports eight men in an emergency shelter setting and 13 families in transitional affordable housing. The Program Director reports to the Executive Director and works closely with the D.S.S. representatives as well as the Resurrection House Inc. staff.
Job Summary:
The Program Director is an integral part of our team who provides supportive and direct intensive case management services to the homeless individuals residing in the shelter as well as the families living in our transitional housing.
The Program Director is responsible for duties, which include, but are not limited to the following:- Conducts initial intake and orientation with new clients.
- Complete Suffolk County Department of Social Services documentation.
- Engages and reinforces the clients in discussions about Residence rules and regulations including chores.
- Provides follow-up and resolution to all client incident reports and complaint reports as required.
- Maintain professional relationships with clients and respects client confidentiality.
- Creates and maintains client files.
- Develops Independent Living Plans with clients and periodic reassessments.
- Conducts intensive case management including collaboration with outside agencies to secure wrap-around services for clients.
- Responsible for making referrals to necessary resources, advocating for clients, and monitoring individual needs.
- Monitor clients’ progress in both Shelter and Transitional Families toward achieving their goals via face-to-face meetings or scheduled telephone contact and document via progress notes.
- Provides all required information for weekly/monthly/quarterly/annual reports.
- Escorts clients to appointments (educational, medical, social service, etc) as needed.
- Enhance individual’s daily living skills.
- Assists in permanent housing search and completing housing packets.
- Completes Discharges.
- Maintains client binders.
- Keeps supervisor informed of all program and client developments.
- Follow up with clients for a period of time after the successful completion of their primary goals to assure client stability.
- Organizes third-party informational sessions.
- Supervises Shelter Support Staff.
- Schedules Shelter Support Staff to ensure 24/7 coverage.
- Must be available to be on call as required Performs other duties as assigned.
