Job Summary:

CPSFL is seeking a dynamic Targeted Case Manager (Wraparound Facilitator). The core function of a Wraparound Facilitator is to monitor, plan, link and advocate for their client and the client’s family. A Wraparound Facilitator should be available to assist the families fold the needed services - including a full range of traditional and non-traditional mental health services, family support, health, education, and social services - into a family-sensitive service plan.

• A bachelor’s degree from an accredited university or college, with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field.
• A minimum of one year of full time or equivalent experience working with children with a serious emotional disturbance.
• Have a Florida Driver’s License and access to transportation.
• Bilingual a plus.

Compensation and Benefits:

• Compensation: $39,620 - $40,620 (Range based on bi-lingual.)
