    Saint Augustine Youth Services (SAYS) has been providing quality therapeutic services to youth for over 30 years and is currently accepting applications for a Targeted Case Manager. This is a unique opportunity to become a member of a dedicated and professional team passionate about the care of the youth, ages 25 and under, that we serve.

    Case managers work with youth and their families to help assess their needs and goals. They then work together with collaborative supports to achieve those goals.

    COMPETENCIES/QUALIFICATIONS: To be certified as a mental health targeted case manager for the children’s mental health target group, an individual must meet the following criteria:

    • Have a bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, or a related human services field and have a minimum of one year of full time or equivalent experience working with children with serious emotional disturbances; or have a bachelor’s degree from an accredited university or college and three years full time or equivalent experience working with children with serious emotional disturbances.
    • Has completed or agrees to complete the Certified Behavioral Health Case Manager (CBHCM) through the FL Certification Board within thirty days of onboarding. Pending on availability of classes from external sources.
    • Has completed or agrees to complete AHCA-approved mental health targeted case management training within three months of initially providing Medicaid services.
    • The ability to communicate effectively, to other members of the treatment team, difficulties that children may be experiencing.
    • Two years’ experience working with children, adolescents, young adults and families
    • A valid Florida driver’s license
    • A history free of offenses as listed on the Affidavit of Good Moral Character
    • Must be able to pass an extensive background screening
    • Must be able to pass a drug screening

    RESPONSIBILITIES: The Targeted Case Manager is responsible:

    • To the COACHES Supervisors.
    • For performing 40 hours of work weekly, which will include flexible hours, including weekends, according to the children’s needs.
    • For conducting the initial assessment and developing the recipients service plan according to Florida Medicaid guidelines.
    • For working with the recipient and the recipient’s family to address issues related to the implementation of the service plan.
    • For assessing the effectiveness of the service plan in meeting the identified needs of the recipient.
    • For linking and facilitating the recipient with appropriate services and resources identified in the service plan through referrals to reach desired goals.
    • For driving clients when necessary to appointments or resource locations in a SAYS vehicle.
    • For advocating for the acquisition of services and resources necessary to implement the service plan by representing or defending recipients through direct intervention.
    • For coordinating the delivery of services as specified in the service plan with the help of the recipient, the recipient’s family, and the recipient’s natural support system.
    • For monitoring service delivery to evaluate the recipient’s progress.
    • For documenting mental health targeted case management activities in accordance with Florida Medicaid documentation requirements.
    • For crisis intervention/support by assisting recipients in crisis in getting access to the necessary resources to cope with the situation.
    • For arranging for and coordinating discharge and after care services when applicable.
    • Assist with fundraising opportunities.

    QUALIFICATIONS/COMPETENCIES:

    QUALIFICATIONS:

    · Practitioner of the healing arts with a Master's degree from an accredited college/university in a major in the field of mental health counseling, social work, marriage and family counseling, or psychology

    · Two years’ experience working with children, adolescents, young adults and families

    • A valid Florida driver’s license
    • A history free of offenses as listed on the Affidavit of Good Moral Character
    • Must be able to pass an extensive background screening
    • Must be able to pass a drug screening

    COMPETENCIES:

    • Present oneself in a positive, professional manner to the children and other agency personnel
    • Must possess excellent oral and written communication skills
    • Be a passionate role model and remain a positive mentor for children
    • Individuals that enjoy working with a team and thinking outside the box to support children and their families.

    RESPONSIBILITIES:

    • For acting as the primary therapist for up to 12 youth.
    • For providing individual, group and family therapy as directed by the individualized treatment plan of each child.
    • For utilizing the Wraparound Model in collaboration with Case Manager, and the family and identified natural supports.
