Captain/Pilot in Command (Large Jet) provides leadership and management of pilots, schedules, and records to maintain efficient and safe flight operations, comply with all FAA regulations, and deliver operational performance on company goals. Maintains the required FAA records and documentation for certifications, crews, flight times, aircraft, and activity. Being a Captain/Pilot in Command (Large Jet) keeps informed about current aviation developments and regulations that impact flight operations. Coordinates pilot training and development plans to ensure all are ready for flight duty. Additionally, Captain/Pilot in Command (Large Jet) uses the Safety Management System (SMS) to identify, mitigate, and manage risk in operations. May require a bachelor's degree. Requires Airline Transport Pilot (ATP) Certificate. Requires FAA Commercial Pilot's License. Typically reports to a director or head of a unit/department. The Captain/Pilot in Command (Large Jet) manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions.     The Patient Navigator will assist patients and their families in managing through the vast array of services and programs. The Patient Navigator will tailor services to the unique wellness needs of the patients served. The Patient Navigator will participate in health screenings, provide patient education, and offer appropriate referrals. The Patient Navigator will be responsible for cultivating relationships with patients and their families, physician partners, members of the healthcare team, and community partners. The Patient Navigator will be responsible for the collection of data related to contacts and referrals as outlined by the department manager. The navigator will be engaged in discussing strategies to continually improve patient and provider experience. The Patient Navigator will be involved in the identification and support of the development of programs and services to enhance patient care.

    Duties:

    • Serves as an advocate to patient/caregiver. Bridges gap to assure the patient has the information, services, and resources they and their family needs.
    • Assist in training and onboarding of additional caregivers.
    • Serves as a source of information and support for patients regarding holistic care including physical, psychosocial, emotional, cultural sensitivity, language barriers, financial challenges, and spiritual support.
    • The patient navigator is responsible to facilitate, organize, and coordinate clinical activities associated with the assigned clinical program.
    • Develops, maintains, and reports metrics for navigation services and programs. Tracks, monitors, and helps prioritize allocated program funds as appropriate. Delegates appropriate tasks and projects to the resource team.
    • Ensures health screening activities including assessments, health profiles, vital sign checks, and lab draws are performed as needed. Acts as a resource for staff. Recommends staff education needs to manager. Performs in-services as needed.
    • Follow established protocols and procedures.
    • Demonstrates responsibility and accountability for own professional practice in regard to: Attendance, punctuality, and patient confidentiality.
    • Positive public relation image and working relationships.
    • Obtaining necessary knowledge and skill needed to maintain competence and certifications.
    • Completing all mandatory requirements, including annual safety programs, CPR, infection prevention and annual physical.

    Qualifications:

    • Bachelor's degree in Social Work
    • At least 1 year of experience in a hospital or clinical setting is highly desirable.
    • Knowledge of disease process, related treatment protocols and common side effects and management.
    • Ability to think critically, synthesize information, and communicate patient care needs in an organized and fluid manner.
    • Ability to work independently and collaboratively, triage patients and keep care team informed of patient status.
    • Possesses basic computer skills: Microsoft Word, Excel, and Outlook.
    • Ability to handle multiple tasks and appropriately prioritize.
    • Ability to clearly communicate, both orally and in writing.
    • Ability to maintain patient confidentiality and privacy.
    • Ability to deal effectively with many people across a broad range of cultural backgrounds and ages.
    • Ability to give constructive feedback, work with teams, teach staff and patients.
    • Create and sustain respectful, inclusive, positive working relationships.
    • Possesses flexibility in adapting to new and challenging situations.
