The Patient Navigator will assist patients and their families in managing through the vast array of services and programs. The Patient Navigator will tailor services to the unique wellness needs of the patients served. The Patient Navigator will participate in health screenings, provide patient education, and offer appropriate referrals. The Patient Navigator will be responsible for cultivating relationships with patients and their families, physician partners, members of the healthcare team, and community partners. The Patient Navigator will be responsible for the collection of data related to contacts and referrals as outlined by the department manager. The navigator will be engaged in discussing strategies to continually improve patient and provider experience. The Patient Navigator will be involved in the identification and support of the development of programs and services to enhance patient care.
