This position can be filled by a Registered Nurse, Occupational Therapist, Physical Therapist or Exercise Physiologist.

The person serving as Cardiac Rehab Specialist is responsible for implementing St. Gabriel's Hospital Cardiac Rehab program.  The Cardiac Rehab Specialist communicates with patients, families, physicians, and other professionals to establish individual therapy programs and provides total patient education and care in Cardiac rehabilitation. Along with the Director of Rehab Services the Cardiac Rehab Specialist plans for all functions of Cardiac Rehab including direct patient services, patient schedules, written standards and regulatory requirements.  The Cardiac Rehab Specialist serves in a multidisciplinary capacity providing general services to a wide variety of populations including pediatric, medical, surgical, ambulatory, and orthopedics.

Essential Key Job Responsibilities

• Patient care including evaluation and treatment of patients referred by providers

• Provides appropriate supervision for department support personnel

• Completes documentation related to patient care in a timely manner

• Participates in and provides educational in-services for staff and providers

• Collaborates with healthcare team and provides professional recommendations regarding patient care

• Participates in patient related care coordination activities (e.g. care conferences, etc.)

• Positive promotion of department and profession within hospital/clinic as well as community

• Other duties as assigned by management

A four (4) year degree from an accredited college in a related filed is required; examples would include Registered Nurse, Physical Therapist, Occupational Therapist and Exercise Science/Physiology

Licensed in the State of Minnesota consistent with field of degree

ACLS certified within 6 months of start of employment

Preferred

One year of experience with cardiac rehab

Specialized post-degree education and training with emphasis on cardiac rehabilitation is highly desirable

Successful experience in delivering quality adult education programs
