Case Management Director oversees a staff of case managers responsible for patient care coordination. General Description: The primary responsibility of this position is to be a leader to the Case Management team and provide onboarding support, guidance, and supervision to Case Managers (CM’s) as it relates to daily CM tasks, treatment plan deadlines, weekday medication deliveries, on-call scheduling, and other tasks as assigned. In addition, this position will provide culturally responsive, trauma informed direct case management support to persons served. This position works directly with the CCS Director/MHP on leading the CCS/TCM/Crisis Stabilization/Ch.35 programs and training staff as needed.
