Case Management Director oversees a staff of case managers responsible for patient care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation. Being a Case Management Director evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Seeks treatments that balance clinical and financial concerns with the family's needs and the patient's quality of life. Additionally, Case Management Director contributes to the development and improvement of clinical care pathways that enhance cost effectiveness while providing quality care. Typically requires a bachelor's degree. Typically reports to top management. May require Registered Nurse (RN). The Case Management Director typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments. To be a Case Management Director typically requires 3+ years of managerial experience.     CLM provides the area's most comprehensive array of mental health services for children and their families. The Case Management Benefits Specialist provides outpatient mental health services including assessment, treatment and care planning, resource identification, benefit coordination, service monitoring, and community-based functional support and counseling. This is a full time, 40 hour per week position.

    Major Functional Areas and Description of Duties

    • Provide new and established clients with benefits support services to successfully initiate and follow through with the benefits application and redetermination process;
    • Provide education and support to educate and create awareness of benefit resources such as but not limited to Aid to the Permanent and Totally Disabled (APTD), Women, Infant and Children’s (WIC) Nutrition Program, Social Security, and Medicaid;
    • Assist clients setting up NH Easy accounts;
    • Support clients and staff requesting NHEASY CLM access to view and/or manage their Medicaid and other DHHS benefits online;
    • Provide ongoing support to assist clients with managing their benefits, reporting required changes including but not limited to income, household and demographic data, and access available work incentives;
    • Work collaboratively with members of the treatment team, Intake Clinicians, Social Security Administration, Department of Health and Human Services (DHHS), clients, natural supports, and other community partners to ensure a wrap-around approach is provided for continuity of client care;
    • Engage in appropriate consultation meetings and committees, both internal and external, as determined by supervisor;
    • Provide overall management of the spenddown population including but not limited to entering spenddown amounts in EHR, monitoring internal and external expenses incurred towards spenddown, timely submission of invoices to DHHS; monitoring retro-active eligibility, billing and eligibility status for spenddown cases.
      • Coordinate and ensure collection of all necessary verifications through case managers, FSS workers, and clinical staff on a timely manner.
      • Ensure submission of complete application with required verifications to DHHS.
      • Notify supervisor of all applications and/or verifications that are not completed/submitted within the required time frame.
    • Function professionally and in a manner that protects the integrity, confidentiality and rights of all clients;
    • Execute all required services and timely documentation in accordance with agency policies and federal/state standards;
    • Maintain professional memberships, licenses, accreditation, certifications, etc.;
    • Represent CLM in a professional and appropriate manner in all settings;
    • Perform all other duties as assigned by supervisor.

    Qualifications and Skills

    • Bachelor’s degree preferred however an Associate’s degree may qualify;
    • Two years of full-time direct care experience provided;
    • Proficient computer skills and knowledge of all Microsoft office applications (Word, Excel, Publisher, Power Point, Outlook, TEAMS, etc.);
    • Ability to navigate an electronic health record;
    • Ability and desire to work as part of a team while also able to be self-directed and motivated with minimal supervision;
    • Ability to acquire and effectively practice new skills as needed according to evolving agency needs/services;
    • Effective communication skills, organizational skills, and ability to multi-task.

    Physical Demands and Working Conditions

    The majority of the work will be office based and/or remote with meetings via TEAMS. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Must be able to flex schedule to accommodate needs of clients and staff. Reliable internet access is required. Reliable transportation to get to the office is required.
