The RN Director of Case Management will be an experienced case management nursing leader, preferably with a certification in case management. As the Nurse Director of Case Management, you will play a pivotal role in ensuring the seamless coordination of patient care throughout the hospital. The RN Case Management Director will coordinate and supervise other social work professionals, guiding them in tasks such as psychosocial assessments, crisis intervention, and discharge planning. They will oversee the strategic planning and assessment of departmental activities, and initiate care management performance improvement goals.
