Now Hiring – Director of Case Management – CareOne Hospital Division

The Director of Case Management will be responsible for managing all aspects of case management operations from admission to discharge, with a focus on optimizing patient care transitions and promoting cost efficiencies. The Director works collaboratively with patients, families, physicians and the interdisciplinary team to ensure goal directed, high quality care to promote positive patient outcomes. This role encompasses oversight of utilization review (UR) discharge planning, and management of length of stay through interdisciplinary team collaboration. The Director will maintain compliance with all Federal, State, and Joint Commission standards.

The Director will manage case management for our 3 LTAC (Long Term Acute Care) locations:

• CareOne at St. Peter’s- New Brunswick, NJ
• CareOne at Trinitas- Elizabeth, NJ
• CareOne at Hackensack University Medical Center- Westwood, NJ

Qualifications:

• Bachelor’s degree in nursing (BSN) required; Master's degree in Nursing (MSN) or related field preferred.
• Registered Nurse (RN) licensed in New Jersey.
• Minimum of 3 to 5 years of clinical hospital experience required.
• Minimum of 3 to 5 years of experience in case management preferred with demonstrated leadership responsibilities.
• Strong knowledge of healthcare regulations including CMS, State and Joint Commission standards.
• Strong knowledge of payer reimbursement models.
• Excellent interpersonal, communication, and leadership skills.
• Proven ability to analyze data, implement quality improvement initiatives, and drive organizational change.
