Case Management Director oversees a staff of case managers responsible for patient care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation. Being a Case Management Director evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Seeks treatments that balance clinical and financial concerns with the family's needs and the patient's quality of life. Additionally, Case Management Director contributes to the development and improvement of clinical care pathways that enhance cost effectiveness while providing quality care. Typically requires a bachelor's degree. Typically reports to top management. May require Registered Nurse (RN). The Case Management Director typically manages through subordinate managers and professionals in larger groups of moderate complexity.     Job Summary and Qualifications
    • Schedule details: This position includes Weekends, holidays and Trauma Call shift requirements with no exceptions.

    ** This position requires 6 weeks of full-time training. The first 3 weeks would be Monday - Friday, 8:00 AM - 5:00 PM. Weeks 4 -6 would be a combinations of evenings, weekends, and days about 8 hours per day.**

    As a Social Worker in Case Management at our facility you will be joining a team that has a passion for creating positive patient interactions and a dedication to caring for the multi-faceted wellness of those we serve. In this role you will:


    • Be responsible for the day-to-day direction of social service patient care, including quality care of the patient from admission to discharge.
    • Evaluate the psychosocial needs of patients and family support systems and coordinate appropriate discharge plans for identified patient populations.
    • Act as a liaison between the facility and resources external to the organization.
    • Implement timely discharge plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care.
    • Provide crisis intervention and support.
    • Assist the Case Manager in facilitating patient movement across the continuum of care and identify and track barriers to patient throughput.
    • Establish and maintain effective cooperative working relationships with social service staff, nursing staff, medical staff, and other departments.

    Qualifications:

    • Bachelor's degree in Social Work required; Master's degree preferred.
    • One of the following current State of Utah Social Worker licenses: Social Service Worker (SSW), Certified Social Worker (CSW), or Licensed Clinical Social Worker (LCSW).
    • 2 or more years of clinical experience preferred.
    • 2 or more years of clinical experience preferred.
    • Knowledge of Utilization Review, DRG review, and Discharge Planning preferred.
