Case Management Director oversees a staff of case managers responsible for patient care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation. Being a Case Management Director evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Seeks treatments that balance clinical and financial concerns with the family's needs and the patient's quality of life. Additionally, Case Management Director contributes to the development and improvement of clinical care pathways that enhance cost effectiveness while providing quality care. Typically requires a bachelor's degree. Typically reports to top management. May require Registered Nurse (RN). The Case Management Director typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments. To be a Case Management Director typically requires 3+ years of managerial experience.   • POSITION SUMMARY:

    The Supported Housing (SH) Case Manager is responsible for providing services for participants in Compass's Supportive Housing program. Primary responsibilities include; providing case management services and life skill development for formerly homeless individuals and families. The case manager's primary responsibility is to support clients in increasing their financial stability, improving their wellness, and reaching greater self-determination. This role will work as part of a team to provide services.

    Responsibilities and Tasks:
    • Conduct assessments and interviews with clients to determine eligibility.
    • Develop and update action plans, case notes, and other documents related to a client's file.
    • Provide ongoing life skills development to assist the client in improving independence and quality of life.
    • Provide specialized and targeted interventions when clients are at risk of eviction.
    • Connect clients to medical, mental health, and other supportive counseling services. Establish a network of community services and conduct appropriate referrals to clients.
    • Guide clients through the process of application, qualification, re-determination for SSI, SSDI, Medicaid, Medicare, SNAP, and other services as needed.
    • Advocate daily.
    • Perform assertive engagement with clients who do not follow through with appointments and case management.
    • Occasionally conduct visits in the client's home.
    • Enlist the support of property management as appropriate for wellness checks.
    • Track and report outcomes. Provide data updates as directed.
    • Attend and actively participate in scheduled internal and external meetings.
    • Travel throughout the area and provide transportation to clients as needed.
    • Represent Compass at professional meetings, community events, and to members of the community.
    • Ability to use assigned technology as directed in the community. Laptop, cell phone and designated software.
    Desired Skill Sets and Other Requirements:
    • Be in possession of a valid driver license and insurance, including safe driving record. Personal transportation required.
    • Ability to pass background and motor vehicle checks
    • Self-starter, excellent time management skills, and proactive problem solver
    • Ability to work a flexible schedule
    • Technology savvy - Especially Microsoft Office Applications and Zoom. PC and Android devices.
    Education and Experience:
    Bachelor's degree (BA/BS) in a social services field; or two years of related experience; or equivalent combination of education and experiencerequired.

    • Complete new-hire training, annual training, attend other workshops and trainings as assigned
    Safety and Health
    • Perform CPR/FA as needed
    • Ability to occasionally lift and/or move up to 50 pounds
